The tax clearance form and all incentive application forms are available to download, save, and print, so you can complete them at your convenience and e-mail, fax or mail them with the required documentation. (Acrobat Reader 5.0 or above is required to use these forms.)
NOTE: Electric Unitary HVAC, Gas Heating and Variable Frequency Drives applications are under development for proposed 2012 changes. Please check this web page for updates.
Steps:

Select a form.
Please select the desired form(s) by clicking on the list at the right.

Complete and print the form. |

When you select a form, one of two things will happen.
- The form will be displayed in your browser window OR
- The browser will ask you to save the form on your desktop as a file. Once you save the file you can open it in Adobe Acrobat Reader.
Print and complete the forms at your convenience. |

Sign and date the form.
Make sure you sign the form before you e-mail, fax or mail it.

Submit the form.
